plans: questionaire | 100 point requirement

All our plan agreements and contracts require 100 points of identification which must include:

At least a current Drivers Licence or Passport

and generally:

Medicare Card

and

ATM Card

or

Credit Card

You must also have evidence of your current address either on a drivers licence or some other official document such as a bank statement or a Government letter.
